Weight Loss

Tirzepatide Injection

Tirzepatide acts on two appetite-regulating hormones, GIP and GLP-1, instead of one, which is why it has become a standout in medical weight management. This compounded weekly injection is planned in three-month programs, with your provider increasing the dose step by step. Compounded medications are not FDA-approved.

Tirzepatide Injection

How Tirzepatide helps with weight loss

Tirzepatide is a first-in-class dual GIP and GLP-1 receptor agonist that works on two separate hunger-regulating pathways at once. That dual action makes it one of the most effective tools available for meaningful, sustained weight loss. Unlike older GLP-1 medications, Tirzepatide activates two separate gut hormone receptors. This dual mechanism has produced some of the highest average weight loss results ever recorded in a clinical obesity trial.

  • Dual-receptor action

By targeting both GIP and GLP-1 receptors, Tirzepatide signals fullness more completely than a single-agonist approach, reducing both hunger and caloric intake.

  • Clinically significant weight loss

In the 72-week SURMOUNT-1 trial of the brand-name medication, adults on the highest dose lost an average of about 21% of their body weight alongside diet and exercise. Compounded versions have not been studied in those trials, and individual results vary.

What to know about Tirzepatide Injection

General education to help you prepare for a conversation with a licensed provider. It is not a prescription or personal medical advice.

Generic name
Tirzepatide
Brand names
Zepbound (weight management and sleep apnea), Mounjaro (type 2 diabetes)
Drug class
Dual GIP and GLP-1 receptor agonist
Form
Once-weekly injection under the skin
Regulatory status
Compounded, not FDA-approved
FDA-approved use
Zepbound: chronic weight management with diet and exercise in adults with obesity, or overweight plus a weight-related condition; also moderate to severe obstructive sleep apnea in adults with obesity.

Brand-name tirzepatide (Zepbound) is FDA-approved for chronic weight management. Tirzepatide prepared by a compounding pharmacy is not FDA-approved, and its strength and inactive ingredients may differ from the brand-name product.

Compounded medications are not FDA-approved. FDA does not review them for safety, effectiveness or quality before they are dispensed. A compounded preparation can differ from approved products in strength, ingredients, dosage form and how it is absorbed, so it should not be assumed to work the same way as an approved medication. Compounding is meant for patients whose needs cannot be met by an FDA-approved drug.

Overview

Tirzepatide is a weekly medication that lowers appetite and food intake. It is used alongside nutrition, activity and behavior changes for long-term weight management.

Who may be evaluated for it

  • Adults with a BMI of 30 or higher, or 27 or higher with a weight-related condition such as high blood pressure, type 2 diabetes or high cholesterol (the criteria used for the FDA-approved weight-management products).
  • People who have tried nutrition and activity changes and want medical support alongside them.

Who may not be a good fit

  • Anyone with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • People who are pregnant, trying to become pregnant or breastfeeding.
  • People with a history of pancreatitis, severe stomach-emptying problems (gastroparesis) or a prior serious allergic reaction to the medication need a careful provider review and are often not candidates.
  • People using GLP-1 medications for cosmetic weight loss without a medical indication.

How it works

Tirzepatide activates two gut-hormone receptors: GLP-1 and GIP. Together they act on brain areas that control hunger and fullness, slow stomach emptying, and help the pancreas release insulin when blood sugar is high.

It lasts about 5 days in the body, which is why it is injected once a week.

What is still uncertain

How much the GIP part adds beyond GLP-1 alone is still being studied. Why response varies so much between people is not fully understood.

Potential benefits and realistic timelines

What the evidence supports

  • In the 72-week SURMOUNT-1 trial of brand-name tirzepatide, adults on the highest dose (15 mg) lost about 21% of body weight on average, versus about 3% with placebo, alongside lifestyle changes.
  • Brand-name tirzepatide reduced breathing interruptions during sleep in adults with obesity and moderate to severe sleep apnea (SURMOUNT-OSA).

Early or limited research

  • Research is ongoing in heart failure, fatty liver disease and kidney disease.

Not established

  • Compounded tirzepatide has not been studied in these trials, so those results cannot be assumed to apply to it.
  • There is no evidence that it targets fat in specific body areas.
  1. First daysSome people notice less appetite within the first week or two, often with mild nausea.
  2. Weeks 4 to 16The dose is usually raised every 4 weeks. Weight loss tends to build gradually during this period.
  3. Around 3 to 6 monthsProviders commonly reassess progress once a maintenance dose has been reached.
  4. After stoppingStudies show much of the lost weight often returns after stopping, so benefits depend on continued treatment and habits.

Results vary from person to person. Your provider decides whether and how long to continue.

How it’s used

For brand-name Zepbound, the label starts at 2.5 mg once weekly for 4 weeks, then increases in 2.5 mg steps at least 4 weeks apart, up to a maximum of 15 mg. Compounded products may be measured differently, so always follow the dose and units on your pharmacy label.

  • Injected under the skin of the abdomen, thigh or upper arm once a week, on the same day each week, with or without food.
  • Rotate injection sites each week.
  • Your provider decides your dose, how fast to increase it and whether to stay at a lower dose.
  • Eating smaller meals, stopping when full, and limiting greasy foods can reduce nausea.

If you miss a dose

Per the Zepbound label: take the missed dose as soon as possible within 4 days (96 hours). If more than 4 days have passed, skip it and take your next dose on the usual day. Do not take two doses within 3 days of each other.

Storage and handling

Store as directed on the pharmacy label. Injectable tirzepatide is generally kept refrigerated, protected from light and not frozen. Never share needles.

Your prescription label and provider instructions always take priority over this general information. Do not change your dose or stop without talking to your provider.

Side effects and safety

More common, usually manageable

  • Nausea
  • Diarrhea or constipation
  • Vomiting
  • Stomach pain and indigestion
  • Decreased appetite
  • Injection-site reactions
  • Tiredness

Less common but serious

  • Inflammation of the pancreas (pancreatitis).
  • Gallbladder problems, including gallstones.
  • Kidney injury, usually from dehydration after vomiting or diarrhea.
  • Low blood sugar, mainly when combined with insulin or a sulfonylurea.
  • Serious allergic reactions.
  • Thyroid C-cell tumors seen in rodent studies (boxed warning on the FDA-approved products); whether this happens in humans is unknown.
  • Slowed stomach emptying, which can raise the risk of inhaling stomach contents during anesthesia or sedation.
  • Depression or thoughts of self-harm (monitored for with weight-management medications).

Get medical help right away

  • Severe stomach pain that does not go away, with or without vomiting, which can spread to your back (possible pancreatitis).
  • Pain in the upper right abdomen, fever, yellow skin or eyes, or clay-colored stools (possible gallbladder problem).
  • A lump or swelling in the neck, trouble swallowing, hoarseness or shortness of breath.
  • Signs of dehydration such as very little urine, dizziness or confusion after ongoing vomiting or diarrhea.

Call 911 or go to the nearest emergency room for chest pain, trouble breathing, swelling of the face or throat, fainting, or signs of a stroke (sudden weakness, numbness, trouble speaking or seeing).

Pregnancy, breastfeeding and fertility

Not for use during pregnancy. Tirzepatide can reduce the effectiveness of birth control pills (see interactions). Stop and contact your provider if you become pregnant. Not recommended while breastfeeding without a provider discussion.

Interactions and combinations

Known interactions

  • Insulin and sulfonylureas (such as glipizide): higher risk of low blood sugar; doses of those medications may need to be lowered.
  • Oral medications in general: slowed stomach emptying can change how other pills are absorbed. Medications with a narrow safety range, such as warfarin or levothyroxine, may need closer monitoring.
  • Other GLP-1 or GIP/GLP-1 medications: should not be combined.
  • Alcohol: can worsen nausea and raise the risk of low blood sugar in people with diabetes.
  • Birth control pills: tirzepatide can make oral contraceptives less effective. The label advises switching to a non-oral method, or adding a barrier method, for 4 weeks after starting and for 4 weeks after each dose increase.

Related treatments and options

Listed for education, not as a recommendation to combine. Any combination needs your provider’s approval.

  • Do not use two GLP-1 based medications together.

  • MetforminOnly with provider supervision

Commonly used together in type 2 diabetes under provider supervision.

  • Insulin or sulfonylureasUse with caution

Raises the risk of low blood sugar; doses are often reduced.

  • Oral birth controlUse with caution

Effectiveness can drop; a backup or non-oral method is advised around dose changes.

Monitoring and follow-up

  • An initial review of your weight history, medical conditions, current medications and family history of thyroid cancer.
  • Baseline labs may be ordered, for example blood sugar (A1c), kidney function or lipids, depending on your history.
  • Regular check-ins during dose increases to review side effects, weight trend, hydration and appetite.
  • People with diabetes may need closer blood sugar monitoring, and a review of retinopathy history.
  • The dose may be held, lowered or stopped if side effects are hard to tolerate or results are not meaningful after a fair trial.

Lab tests and follow-up visits are not included in the medication price unless your plan says so. Your provider will tell you what is needed.

Alternatives to discuss

  • Semaglutide

A weekly GLP-1-only injection with a longer track record, including cardiovascular outcome data for the brand-name product.

  • Tirzepatide Oral Tablet

A compounded tablet for people who prefer to avoid injections. There is no FDA-approved oral tirzepatide, and how well a compounded tablet is absorbed has not been established.

  • Other approved options

Medications such as phentermine-topiramate or naltrexone-bupropion, and for some people metabolic surgery, are other options to discuss.

  • Lifestyle-only approach

Structured nutrition, activity and behavior programs remain the foundation.

Frequently Asked Questions

Is compounded tirzepatide the same as Zepbound or Mounjaro?

No. It contains the same active ingredient but is not FDA-approved and has not been through the same trials or quality review. Strength, inactive ingredients and dosing units can differ.

How is tirzepatide different from semaglutide?

Semaglutide acts on GLP-1 receptors only; tirzepatide acts on both GIP and GLP-1 receptors. In a head-to-head trial of brand-name products, tirzepatide led to more weight loss on average. Side effects are similar.

Does it affect birth control?

It can make birth control pills less effective, especially after starting and after each dose increase. Ask your provider about a backup or non-oral method.

How long will I need to take it?

It is studied as a long-term medication. In trials, many people regained weight after stopping, so plan with your provider for the long term.

Do I need to tell my surgeon or dentist?

Yes. Because tirzepatide slows stomach emptying, tell anyone planning anesthesia or sedation that you take it.

Do I need a prescription?

Yes. After your free consultation, a licensed provider reviews your intake and health history to decide whether this treatment is right for you.

How is Tirzepatide taken?

Tirzepatide is a small subcutaneous injection you give yourself once a week at home. Your provider sets your starting dose and adjusts it over time.

How soon will I see results?

Many patients notice a meaningful reduction in appetite within the first two weeks. Measurable weight changes typically begin within the first month and compound over time.

What are the common side effects?

The most common effects include nausea, reduced appetite, and mild digestive changes, particularly during dose increases. Starting low and titrating gradually significantly reduces their impact.

What if a provider decides this isn't right for me?

Your consultation is free, and no payment is needed to complete the assessment. If a provider decides this treatment isn't appropriate for you, they will explain why and may suggest other options.

How much does it cost?

The consultation is free and you pay nothing to complete the assessment. If a provider approves treatment, you see the exact monthly price for your plan before you are charged. There are no surprise fees.

Can I use my HSA or FSA?

Yes. Medura accepts HSA and FSA cards at checkout, and we can provide documentation for reimbursement if you need it.

How is it shipped?

Your treatment arrives in plain, unbranded packaging with no pharmacy name on the outside. Temperature-sensitive items ship with cold-chain protection.

What happens after I request treatment through Medura?

You complete an online assessment about your health history, current medications and goals. A licensed provider reviews it and decides whether a prescription is appropriate. Completing the assessment does not guarantee a prescription. If one is written, a licensed pharmacy prepares and ships it, and your provider can adjust or stop treatment as needed. If lab work is needed, your provider will tell you; it may be ordered and billed separately.

What to expect and possible side effects

  1. 1

    Two hormones, one injection

Tirzepatide activates both GIP and GLP-1 receptors, targeting appetite from two distinct biological pathways, something no single-agonist treatment can match.

  2. 2

    Gradual dose titration

You start low and increase on a schedule your provider controls. This keeps side effects manageable while your body adapts.

  3. 3

    Results that compound

Most patients notice reduced hunger and improved satiety in the first weeks. Significant body weight changes build over months with consistent dosing.

Common side effects:

  • Nausea (most common, especially early in treatment, typically improves)
  • Decreased appetite (expected and therapeutic)
  • Mild diarrhea or constipation
  • Occasional vomiting or stomach discomfort
  • These typically subside as your body adjusts to the medication
