Tadalafil tablets are FDA-approved for ED (as needed and once daily) and for urinary symptoms of an enlarged prostate (BPH). The 5 mg daily tablet is an approved strength. A 25 mg capsule is a compounded strength above the 20 mg maximum on the FDA label and is not FDA-approved.

Compounded medications are not FDA-approved. FDA does not review them for safety, effectiveness or quality before they are dispensed. A compounded preparation can differ from approved products in strength, ingredients, dosage form and how it is absorbed, so it should not be assumed to work the same way as an approved medication. Compounding is meant for patients whose needs cannot be met by an FDA-approved drug.

Overview

Tadalafil helps men with ED get and keep an erection with sexual stimulation. Daily low-dose use can also ease urinary symptoms of an enlarged prostate.

Who may be evaluated for it

  • Men with erectile dysfunction (trouble getting or keeping an erection firm enough for sex).
  • Men whose heart health allows sexual activity, as assessed by a provider.

Who may not be a good fit

  • Anyone taking nitrates in any form (nitroglycerin, isosorbide) or recreational “poppers” (amyl nitrite): the combination can cause a dangerous drop in blood pressure.
  • Anyone taking riociguat (Adempas).
  • People told to avoid sexual activity because of heart disease, or who had a heart attack, stroke or life-threatening arrhythmia in the last 6 months.
  • People with very low blood pressure, or a history of sudden vision loss from NAION.

How it works

Like sildenafil, tadalafil blocks the PDE5 enzyme so the chemical that relaxes penile blood vessels lasts longer during arousal.

It stays in the body much longer (about 17.5 hours half-life), which is why its effect can last up to 36 hours and why it can be taken daily.

How it’s used

The FDA label: as needed, usually 10 mg (range 5 to 20 mg) before sex, not more than once a day; or once daily 2.5 to 5 mg. Your prescription sets your dose.

  • As needed: take at least 30 minutes before sexual activity, no more than once a day.
  • Daily: take at the same time every day regardless of sexual activity.
  • Can be taken with or without food.

If you miss a dose

Daily use: take it when you remember the same day; do not take two doses in one day. As-needed use has no missed doses.

Side effects and safety

More common, usually manageable

  • Headache
  • Indigestion
  • Back pain or muscle aches
  • Flushing
  • Stuffy nose

Less common but serious

  • Sudden vision loss in one or both eyes (non-arteritic anterior ischemic optic neuropathy, NAION), which is rare.
  • Sudden hearing loss or ringing in the ears.
  • An erection lasting more than 4 hours (priapism), which can permanently damage the penis.
  • A large drop in blood pressure, especially with nitrates or alpha-blockers.

Get medical help right away

  • Chest pain during or after sex: stop and seek emergency care. Tell responders you took a PDE5 inhibitor, because nitrates must not be given.
  • An erection lasting more than 4 hours.
  • Sudden loss of vision or hearing.

Call 911 or go to the nearest emergency room for chest pain, trouble breathing, swelling of the face or throat, fainting, or signs of a stroke (sudden weakness, numbness, trouble speaking or seeing).

Pregnancy, breastfeeding and fertility

Not intended for women in this use. Sildenafil is not a fertility treatment and does not protect against pregnancy or infections.

Interactions and combinations

Known interactions

  • Nitrates and “poppers”: never combine.
  • Alpha-blockers (doxazosin, tamsulosin, terazosin) and other blood pressure medicines: can add to blood pressure lowering. Your provider may start at a low dose.
  • Strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir, clarithromycin) raise levels; lower doses or longer gaps are needed.
  • Alcohol: heavy drinking adds to blood pressure lowering and can worsen erections.
  • Grapefruit juice can raise levels.
  • Because tadalafil lasts long, nitrates must be avoided for at least 48 hours after a dose.
