Sildenafil tablets are FDA-approved for erectile dysfunction in 25, 50 and 100 mg strengths. A 55 mg capsule is a compounded strength and is not FDA-approved.
Compounded medications are not FDA-approved. FDA does not review them for safety, effectiveness or quality before they are dispensed. A compounded preparation can differ from approved products in strength, ingredients, dosage form and how it is absorbed, so it should not be assumed to work the same way as an approved medication. Compounding is meant for patients whose needs cannot be met by an FDA-approved drug.
Overview
Sildenafil helps men with ED get and keep an erection when sexually aroused. It does not cause an erection on its own and does not increase desire.
Who may be evaluated for it
- Men with erectile dysfunction (trouble getting or keeping an erection firm enough for sex).
- Men whose heart health allows sexual activity, as assessed by a provider.
Who may not be a good fit
- Anyone taking nitrates in any form (nitroglycerin, isosorbide) or recreational “poppers” (amyl nitrite): the combination can cause a dangerous drop in blood pressure.
- Anyone taking riociguat (Adempas).
- People told to avoid sexual activity because of heart disease, or who had a heart attack, stroke or life-threatening arrhythmia in the last 6 months.
- People with very low blood pressure, or a history of sudden vision loss from NAION.
How it works
During arousal, nerves release nitric oxide, which raises a chemical called cGMP in the penis. cGMP relaxes blood vessels so blood can flow in and create an erection.
The enzyme PDE5 breaks down cGMP. Sildenafil blocks PDE5, so cGMP lasts longer and blood flow is easier to maintain. Sexual stimulation is still needed.

How it’s used
The FDA label for ED: usually 50 mg about 1 hour before sex, adjusted to 25 to 100 mg, no more than once a day.
- Take about 30 minutes to 1 hour before sexual activity.
- Taking it on an empty stomach or after a light meal helps it work faster.
- No more than one dose in 24 hours.
If you miss a dose
It is taken as needed, so there is no missed dose. Never take more than one dose in 24 hours.

Side effects and safety
More common, usually manageable
- Headache
- Flushing
- Stuffy nose
- Indigestion
- Dizziness
- Blue-tinted or blurred vision, or light sensitivity
Less common but serious
- Sudden vision loss in one or both eyes (non-arteritic anterior ischemic optic neuropathy, NAION), which is rare.
- Sudden hearing loss or ringing in the ears.
- An erection lasting more than 4 hours (priapism), which can permanently damage the penis.
- A large drop in blood pressure, especially with nitrates or alpha-blockers.
Get medical help right away
- Chest pain during or after sex: stop and seek emergency care. Tell responders you took a PDE5 inhibitor, because nitrates must not be given.
- An erection lasting more than 4 hours.
- Sudden loss of vision or hearing.
Call 911 or go to the nearest emergency room for chest pain, trouble breathing, swelling of the face or throat, fainting, or signs of a stroke (sudden weakness, numbness, trouble speaking or seeing).
Pregnancy, breastfeeding and fertility
Not intended for women in this use. Sildenafil is not a fertility treatment and does not protect against pregnancy or infections.
Interactions and combinations
Known interactions
- Nitrates and “poppers”: never combine.
- Alpha-blockers (doxazosin, tamsulosin, terazosin) and other blood pressure medicines: can add to blood pressure lowering. Your provider may start at a low dose.
- Strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir, clarithromycin) raise levels; lower doses or longer gaps are needed.
- Alcohol: heavy drinking adds to blood pressure lowering and can worsen erections.
- Grapefruit juice can raise levels.
