What to know about Olympus RDT
General education to help you prepare for a conversation with a licensed provider. It is not a prescription or personal medical advice.
- Generic name
- Oxytocin, tadalafil and bremelanotide
- Drug class
- Compounded combination (hormone, PDE5 inhibitor and melanocortin agonist)
- Form
- Troche dissolved in the cheek or under the tongue
- Regulatory status
- Compounded, not FDA-approved
This compounded troche combines three ingredients. It is not FDA-approved, and the combination has not been studied in clinical trials for safety or effectiveness. Tadalafil and bremelanotide are each FDA-approved in other forms for specific uses; oxytocin is FDA-approved only as an injection used in childbirth.
Compounded medications are not FDA-approved. FDA does not review them for safety, effectiveness or quality before they are dispensed. A compounded preparation can differ from approved products in strength, ingredients, dosage form and how it is absorbed, so it should not be assumed to work the same way as an approved medication. Compounding is meant for patients whose needs cannot be met by an FDA-approved drug.
Overview
A compounded combination troche (oxytocin 100 IU, tadalafil 5 mg, bremelanotide 2 mg) some providers prescribe for male sexual function. Its benefit as a combination has not been established.
Who may be evaluated for it
- Adults with sexual function concerns who have discussed approved options with a provider.
Who may not be a good fit
- Anyone taking nitrates in any form (nitroglycerin, isosorbide) or recreational “poppers” (amyl nitrite): the combination can cause a dangerous drop in blood pressure.
- Anyone taking riociguat (Adempas).
- People told to avoid sexual activity because of heart disease, or who had a heart attack, stroke or life-threatening arrhythmia in the last 6 months.
- People with very low blood pressure, or a history of sudden vision loss from NAION.
- People with uncontrolled high blood pressure (bremelanotide raises blood pressure).
- Anyone who is pregnant.
How it works
Tadalafil improves blood flow during arousal by blocking PDE5.
Bremelanotide (PT-141) acts on melanocortin receptors in the brain involved in desire.
Oxytocin is a hormone involved in bonding and orgasm. Whether it is absorbed meaningfully through a troche, and whether it adds anything, is not known.
What is still uncertain
How much of each ingredient is absorbed from a troche, and whether the three interact, has not been studied.
Potential benefits and realistic timelines
What the evidence supports
- Each of tadalafil and bremelanotide has evidence on its own in its approved form.
Not established
- Any benefit of this specific combination.
- Absorption of oxytocin or bremelanotide through the cheek.
- Long-term safety.
- UnknownNo clinical studies define when this combination works. Follow your provider’s instructions.
Results vary from person to person. Your provider decides whether and how long to continue.
How it’s used
- Place in the cheek or under the tongue and let it dissolve fully; do not chew or swallow whole unless directed.
- Use as needed, no more than once in 24 hours unless directed.
If you miss a dose
Used as needed; never exceed the frequency on your label.
Storage and handling
Store as on the pharmacy label; some troches require refrigeration.
Your prescription label and provider instructions always take priority over this general information. Do not change your dose or stop without talking to your provider.
Side effects and safety
More common, usually manageable
- Headache
- Flushing
- Nausea
- Stuffy nose
- Indigestion
Less common but serious
- Sudden vision loss in one or both eyes (non-arteritic anterior ischemic optic neuropathy, NAION), which is rare.
- Sudden hearing loss or ringing in the ears.
- An erection lasting more than 4 hours (priapism), which can permanently damage the penis.
- A large drop in blood pressure, especially with nitrates or alpha-blockers.
- Blood pressure rise from bremelanotide
- Skin darkening with frequent bremelanotide use
Get medical help right away
- Chest pain during or after sex: stop and seek emergency care. Tell responders you took a PDE5 inhibitor, because nitrates must not be given.
- An erection lasting more than 4 hours.
- Sudden loss of vision or hearing.
Call 911 or go to the nearest emergency room for chest pain, trouble breathing, swelling of the face or throat, fainting, or signs of a stroke (sudden weakness, numbness, trouble speaking or seeing).
Pregnancy, breastfeeding and fertility
Not for use during pregnancy. Oxytocin can stimulate uterine contractions.
Interactions and combinations
Known interactions
- Nitrates and “poppers”: never combine.
- Alpha-blockers (doxazosin, tamsulosin, terazosin) and other blood pressure medicines: can add to blood pressure lowering. Your provider may start at a low dose.
- Strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir, clarithromycin) raise levels; lower doses or longer gaps are needed.
- Alcohol: heavy drinking adds to blood pressure lowering and can worsen erections.
- Grapefruit juice can raise levels.
- Oral naltrexone (bremelanotide can make it less effective).

Can I use my HSA or FSA?
Yes. Medura accepts HSA and FSA cards at checkout, and we can provide documentation for reimbursement if you need it.
What happens after I request treatment through Medura?
You complete an online assessment about your health history, current medications and goals. A licensed provider reviews it and decides whether a prescription is appropriate. Completing the assessment does not guarantee a prescription. If one is written, a licensed pharmacy prepares and ships it, and your provider can adjust or stop treatment as needed.
