What to know about PT-141 Injectable

General education to help you prepare for a conversation with a licensed provider. It is not a prescription or personal medical advice.

Generic name
Bremelanotide (PT-141)
Brand names
Vyleesi (FDA-approved autoinjector)
Drug class
Melanocortin receptor agonist
Form
Injection under the skin as needed
Regulatory status
Compounded, not FDA-approved
FDA-approved use
Vyleesi: acquired, generalized HSDD in premenopausal women not caused by a medical or psychiatric condition, relationship problems or medications.

Bremelanotide is FDA-approved as Vyleesi, a 1.75 mg autoinjector for premenopausal women with acquired, generalized hypoactive sexual desire disorder (HSDD). A compounded vial is not FDA-approved. Use in men or postmenopausal women is off-label.

Compounded medications are not FDA-approved. FDA does not review them for safety, effectiveness or quality before they are dispensed. A compounded preparation can differ from approved products in strength, ingredients, dosage form and how it is absorbed, so it should not be assumed to work the same way as an approved medication. Compounding is meant for patients whose needs cannot be met by an FDA-approved drug.

Overview

Bremelanotide is used to address low sexual desire that causes distress. It works in the brain rather than on blood flow.

Who may be evaluated for it

  • Premenopausal women with low sexual desire that causes distress and is not explained by another condition (the approved use).

Who may not be a good fit

  • People with uncontrolled high blood pressure or known heart disease.
  • Anyone who is pregnant.
  • People taking oral naltrexone for alcohol or opioid use.

How it works

Bremelanotide activates melanocortin receptors (mainly MC4R) in the brain, which are involved in sexual desire and arousal. It does not act on blood flow the way ED pills do.

What is still uncertain

Exactly how melanocortin activation increases desire is not fully understood.

Potential benefits and realistic timelines

What the evidence supports

  • In trials of Vyleesi, premenopausal women with HSDD reported a modest increase in desire and less distress compared with placebo.

Not established

  • Effectiveness in men or postmenopausal women.
  • That compounded bremelanotide performs like Vyleesi.
  1. About 45 minutesVyleesi is injected at least 45 minutes before anticipated sexual activity.
  2. 8 weeksThe label advises stopping if there is no improvement after 8 weeks.

Results vary from person to person. Your provider decides whether and how long to continue.

How it’s used

Vyleesi label: 1.75 mg under the skin at least 45 minutes before sex; no more than 1 dose in 24 hours and no more than 8 doses a month.

  • Inject under the skin of the abdomen or thigh as directed.
  • Use a new sterile needle each time.

If you miss a dose

Used as needed, so there is no missed dose. Never exceed 1 dose in 24 hours.

Storage and handling

Store as directed on the pharmacy label; compounded vials are often refrigerated after opening.

Your prescription label and provider instructions always take priority over this general information. Do not change your dose or stop without talking to your provider.

Side effects and safety

More common, usually manageable

  • Nausea (very common, especially with the first dose)
  • Flushing
  • Headache
  • Injection-site reactions

Less common but serious

  • Temporary rise in blood pressure and drop in heart rate after each dose
  • Darkening of the skin, gums or breasts (focal hyperpigmentation), especially with frequent use; may not go away

Get medical help right away

  • Severe headache, chest pain or palpitations after a dose
  • Persistent vomiting

Call 911 or go to the nearest emergency room for chest pain, trouble breathing, swelling of the face or throat, fainting, or signs of a stroke (sudden weakness, numbness, trouble speaking or seeing).

Pregnancy, breastfeeding and fertility

Stop if pregnant. Use effective birth control while using it.

Interactions and combinations

Known interactions

  • Oral naltrexone: bremelanotide can greatly lower naltrexone levels, which may make it fail.
  • Medicines that need steady absorption (such as some antibiotics) may be affected by slowed stomach emptying.

Related treatments and options

Listed for education, not as a recommendation to combine. Any combination needs your provider’s approval.

Monitoring and follow-up

  • Blood pressure review before starting.
  • Check-in after about 8 weeks to judge benefit.
  • Watch for skin darkening.

Lab tests and follow-up visits are not included in the medication price unless your plan says so. Your provider will tell you what is needed.
