Testosterone Cypionate Injectable
How Testosterone Cypionate supports healthy testosterone care
Testosterone Cypionate is an injectable form of testosterone. Medura offers it at 200 mg/mL as a commercially manufactured multi-dose vial or as a compounded preparation in MCT oil. Testosterone therapy is not one-size-fits-all. Your provider uses your symptoms, history, and lab results to decide whether treatment fits and which formulation makes sense.
Lab-informed decisions
Your provider reviews existing labs or orders new ones to confirm whether low testosterone is present before recommending anything.
Several formulation options
Injectable, topical, and oral options are available through our pharmacy network. Your provider explains why one may suit you better than another.
Ongoing follow-up
Your provider checks in on how you are responding and adjusts your plan over time rather than writing a single prescription and moving on.
Provider-guided from start to finish
A licensed provider reviews your health history, checks for contraindications, sets an appropriate starting plan, and monitors your progress over time.
How to use it
Clear instructions for your specific formulation arrive with your first order.

- Depo-Testosterone
- Drug class
- Androgen (male sex hormone), Schedule III controlled substance
- Form
- Injection into the muscle or under the skin
- Regulatory status

- FDA-approved use
- Testosterone replacement in men with primary or hypogonadotropic hypogonadism.
Commercial testosterone cypionate (in cottonseed oil) is FDA-approved to replace testosterone in men with hypogonadism caused by a testicular, pituitary or hypothalamic condition. It is a controlled substance with federal rules on prescribing and refills.
Overview
Testosterone cypionate restores testosterone to the normal range in men with confirmed low levels, to relieve symptoms such as low libido and fatigue. It is long-term treatment, not a short course.
Who may be evaluated for it
- Men with symptoms of low testosterone (such as low libido, erectile problems, fatigue or loss of muscle) AND consistently low morning testosterone on at least two separate blood tests, as recommended by the Endocrine Society.
- Men with a diagnosed cause of hypogonadism, such as a testicular or pituitary condition.
Who may not be a good fit
- Men with known or suspected prostate cancer or male breast cancer.
- Men actively trying to conceive: testosterone lowers sperm production and can cause infertility.
- Women who are or may become pregnant (testosterone can harm a developing baby).
- Men with a very high red blood cell count (hematocrit), untreated severe sleep apnea, uncontrolled heart failure, or a recent heart attack or stroke need careful review first.
- Men with normal testosterone levels seeking performance or bodybuilding effects.
How it works
Testosterone is the main male sex hormone. It binds to androgen receptors throughout the body, supporting sex drive, erections, muscle and bone strength, red blood cell production and energy.
Taking testosterone from outside the body raises blood levels, but it also signals the brain to stop releasing LH and FSH, the hormones that tell the testicles to make their own testosterone and sperm. That is why sperm counts and testicle size often fall on treatment.
What is still uncertain
Why symptoms respond so differently at similar testosterone levels is not fully understood.
Potential benefits and realistic timelines
What the evidence supports
- In men with confirmed low testosterone, treatment modestly improves sex drive, sexual activity and erectile function (Testosterone Trials, NEJM 2016).
- It can increase bone density and correct mild anemia related to low testosterone.
- Treatment increases lean muscle and reduces fat mass modestly.
Early or limited research
- Effects on mood and energy are smaller and less consistent across studies.
Not established
- Benefits in men with normal testosterone levels.
- Improvement in thinking or memory (not shown in the Testosterone Trials).
- Anti-aging effects or longer life.
- 3 to 6 weeksSex drive changes, if any, are often noticed first.
- 3 to 6 monthsChanges in body composition and energy are typically assessed. Your provider usually checks testosterone and blood counts in this window.
- 6 months and beyondBone density changes take a year or more. If symptoms have not improved by about 6 months at normal levels, treatment is usually reconsidered.
- After stoppingLevels fall back and symptoms tend to return. Natural production may take months to recover.
Results vary from person to person. Your provider decides whether and how long to continue.
How it’s used
The FDA label lists a range of 50 to 400 mg every 2 to 4 weeks. In current practice, providers often use smaller, more frequent doses to keep levels steady. Your dose and schedule are set by your provider based on your lab results.
- Given as an injection into a large muscle (or under the skin if your provider directs), on a regular schedule.
- Use a new sterile needle and syringe each time, and rotate sites.
- Inspect the vial: if crystals have formed in the cold, warming and shaking the vial usually redissolves them. Do not use if it stays cloudy.
If you miss a dose
Take it as soon as you remember, then ask your provider how to adjust the schedule. Do not double the next dose.
Storage and handling
Store at room temperature, protected from light. Keep out of reach of children and dispose of needles in a sharps container.
Your prescription label and provider instructions always take priority over this general information. Do not change your dose or stop without talking to your provider.
Side effects and safety
More common, usually manageable
- Acne or oily skin
- Injection-site pain
- Breast tenderness or enlargement
- Fluid retention
- Mood changes or irritability
- Higher hematocrit on blood tests
Less common but serious
- High red blood cell count (erythrocytosis), which can thicken the blood.
- Increased blood pressure (a warning added to all testosterone labels in 2025).
- Blood clots in the legs or lungs.
- Worsening of sleep apnea or urinary symptoms from an enlarged prostate.
- Reduced sperm count and infertility, and testicular shrinkage.
- Abuse and dependence at high doses, with serious heart, mental health and liver effects.
Get medical help right away
- Pain, swelling, warmth or redness in one leg, or sudden shortness of breath or chest pain (possible blood clot).
- An erection that lasts more than 4 hours.
- Trouble urinating or a sudden inability to urinate.
Call 911 or go to the nearest emergency room for chest pain, trouble breathing, swelling of the face or throat, fainting, or signs of a stroke (sudden weakness, numbness, trouble speaking or seeing).
Pregnancy, breastfeeding and fertility
Testosterone can cause birth defects and must not be used by anyone who is pregnant. In men it suppresses sperm production; men who want children now or soon should discuss fertility-sparing options first.
Interactions and combinations
Known interactions
- Blood thinners such as warfarin: testosterone can increase their effect; more frequent INR checks may be needed.
- Insulin and diabetes medications: testosterone can lower blood sugar, so diabetes medication doses may need adjusting.
- Corticosteroids: may increase fluid retention.
- Anabolic steroids or other androgen products bought outside a prescription should not be combined.
Related treatments and options
Listed for education, not as a recommendation to combine. Any combination needs your provider’s approval.
- HCG (Chorionic)Only with provider supervision
Sometimes added to help maintain sperm production and testicle size. This is off-label and evidence is limited to smaller studies.
- Anastrozole SRUse with caution
Not recommended routinely by the Endocrine Society. Lowering estrogen too far in men can cause joint pain, low libido and bone loss.
Using two testosterone products at once is not recommended unless your provider specifically directs a switch.
Monitoring and follow-up
- Two early-morning testosterone tests before starting to confirm low levels, plus LH/FSH to look for a cause.
- Baseline hematocrit (red cell count), and a PSA (prostate test) for men at the ages recommended by guidelines.
- Testosterone and hematocrit rechecked about 3 to 6 months after starting, then at least yearly.
- Blood pressure checks, and review of sleep, urinary symptoms and mood.
- The dose may be lowered or held if hematocrit rises too high, and treatment stopped if prostate cancer is found.
Lab tests and follow-up visits are not included in the medication price unless your plan says so. Your provider will tell you what is needed.
Alternatives to discuss
- Testosterone Cream
Daily topical option that avoids injections but can transfer to others through skin contact.
- Enclomiphene Citrate
An oral medication that prompts the body to make more of its own testosterone and may preserve sperm. It is not FDA-approved.
- HCG (Chorionic)
Stimulates the testicles directly; FDA-approved for some forms of male hypogonadism and used when fertility matters.
- Lifestyle factors
Weight loss, better sleep, treating sleep apnea and reducing alcohol can raise testosterone in some men.
Lab tested medications for quality and potency.
Every medication ships from a licensed U.S. pharmacy in our network, delivered straight to your door.
Third party quality control testing

Frequently Asked Questions
Will testosterone affect my fertility?
Yes. Testosterone from outside the body usually lowers sperm production, sometimes to zero. It typically recovers after stopping, but this can take many months and is not guaranteed. Tell your provider if you want children.
Does testosterone cause heart attacks?
A large trial (TRAVERSE, 2023) did not find a higher rate of heart attack or stroke in men with low testosterone treated with testosterone gel. In 2025 FDA updated all testosterone labels to reflect this and added a warning about increased blood pressure. Blood clots and high red blood cell counts remain known risks.
Why do I need blood tests?
To confirm your levels are in range and to catch a rising red blood cell count early, before it causes problems. PSA testing may also be part of monitoring.
Can I stop at any time?
Talk to your provider first. Symptoms usually return, and it can take time for your own production to recover.
Do I need a prescription?
Yes. After your free consultation, a licensed provider reviews your intake and relevant lab work to decide whether testosterone therapy is appropriate for you.
Will I need lab work?
Your provider will review existing labs or order new ones to confirm whether low testosterone is present before recommending treatment.
What if a provider decides this isn’t right for me?
Your consultation is free. If a provider decides testosterone therapy isn’t appropriate for you, they will explain why and may suggest other options.
Can I use my HSA or FSA?
Yes. Medura accepts HSA and FSA cards at checkout, and we can provide documentation for reimbursement if you need it.
How is it shipped?
Your treatment arrives in plain, unbranded packaging with no pharmacy name on the outside. Temperature-sensitive items ship with cold-chain protection.
What happens after I request treatment through Medura?
You complete an online assessment about your health history, current medications and goals. A licensed provider reviews it and decides whether a prescription is appropriate. Completing the assessment does not guarantee a prescription. If one is written, a licensed pharmacy prepares and ships it, and your provider can adjust or stop treatment as needed. If lab work is needed, your provider will tell you; it may be ordered and billed separately.
