What to know about T3/T4 Capsule

General education to help you prepare for a conversation with a licensed provider. It is not a prescription or personal medical advice.

Generic name
Liothyronine (T3) and levothyroxine (T4)
Brand names
Armour Thyroid and NP Thyroid are animal-derived T3/T4 products (not FDA-approved); none is a compounded synthetic combination
Drug class
Thyroid hormone
Form
Combination capsule
Regulatory status
Compounded, not FDA-approved

This compounded capsule combines synthetic T3 and T4. It is not FDA-approved. Levothyroxine and liothyronine are each FDA-approved separately. Thyroid hormone labels carry a boxed warning against use for weight loss.

Compounded medications are not FDA-approved. FDA does not review them for safety, effectiveness or quality before they are dispensed. A compounded preparation can differ from approved products in strength, ingredients, dosage form and how it is absorbed, so it should not be assumed to work the same way as an approved medication. Compounding is meant for patients whose needs cannot be met by an FDA-approved drug.

Overview

A single capsule containing both thyroid hormones, used by some providers for hypothyroidism when symptoms persist on T4 alone.

Who may be evaluated for it

  • People with hypothyroidism confirmed by blood tests, usually already treated with levothyroxine.

Who may not be a good fit

  • Anyone without a diagnosed thyroid condition, including for weight loss or fatigue alone.
  • People with untreated adrenal insufficiency.
  • People with an overactive thyroid, recent heart attack or uncontrolled heart rhythm problems need caution.

How it works

T3 is the active form of thyroid hormone. It enters cells and switches on genes that control metabolism, heart rate, temperature and energy use. Most T3 in the body is normally converted from T4.

What is still uncertain

Whether some people benefit from added T3 because of how they convert T4 to T3 is still being researched.

Potential benefits and realistic timelines

What the evidence supports

  • Thyroid hormone replacement corrects hypothyroidism.

Early or limited research

  • Some patients report feeling better on T4 plus T3, but most trials have not shown a consistent advantage over T4 alone.

Not established

  • Weight loss in people with normal thyroid function (and it can be dangerous).
  • Advantages of sustained-release capsules over standard tablets.
  1. DaysT3 acts within hours to days.
  2. 4 to 8 weeksThyroid labs are rechecked after starting or changing the dose.

Results vary from person to person. Your provider decides whether and how long to continue.

How it’s used

  • Take at the same time each day, consistently with or without food.
  • Separate from calcium, iron, antacids and bile-acid binders by at least 4 hours.

If you miss a dose

Take it when you remember unless it is near your next dose. Do not double up.

Storage and handling

Room temperature, away from light and moisture.

Your prescription label and provider instructions always take priority over this general information. Do not change your dose or stop without talking to your provider.

Side effects and safety

More common, usually manageable

  • Symptoms of too much thyroid hormone: palpitations, nervousness, sweating, heat intolerance, trouble sleeping

Less common but serious

  • Irregular heart rhythm (especially atrial fibrillation)
  • Chest pain in people with heart disease
  • Bone loss with long-term overtreatment

Get medical help right away

  • Chest pain, a racing or irregular heartbeat, or fainting

Call 911 or go to the nearest emergency room for chest pain, trouble breathing, swelling of the face or throat, fainting, or signs of a stroke (sudden weakness, numbness, trouble speaking or seeing).

Pregnancy, breastfeeding and fertility

Thyroid needs usually rise in pregnancy. Tell your provider right away if you become pregnant; levothyroxine is usually preferred.

Interactions and combinations

Known interactions

  • Calcium, iron, antacids and cholestyramine reduce absorption.
  • Warfarin: thyroid hormone increases its effect.
  • Diabetes medications: doses may need adjusting.
  • Oral estrogen can raise thyroid hormone needs.

Related treatments and options

Listed for education, not as a recommendation to combine. Any combination needs your provider’s approval.

  • Already contains T3.

  • LevothyroxineAvoid

Already contains T4 unless your provider directs otherwise.

Monitoring and follow-up

  • TSH and free T3/free T4 4 to 8 weeks after starting or any dose change, then periodically.
  • Heart rate and symptom review, especially in older adults or people with heart disease.

Lab tests and follow-up visits are not included in the medication price unless your plan says so. Your provider will tell you what is needed.

Alternatives to discuss

  • Levothyroxine (T4)

    Standard first-line FDA-approved treatment.

  • Liothyronine (T3) SR

T3 alone, usually added to separate T4.

Frequently Asked Questions

How is this different from Armour Thyroid?

Armour Thyroid is made from pig thyroid gland. This capsule is a compounded mix of synthetic T3 and T4. Neither is FDA-approved.

Can thyroid hormone help me lose weight?

No. Labels carry a boxed warning against using thyroid hormone for weight loss in people with normal thyroid function; doses large enough to cause weight loss can cause serious harm.

Why would T3 be added?

Some people with hypothyroidism still have symptoms on T4 alone. Adding T3 is an option some providers try, though trials have shown mixed results.

Why separate it from calcium or iron?

They bind thyroid hormone in the gut and reduce how much you absorb.

Do I need a prescription?

Yes. After your free consultation, a licensed provider reviews your intake, symptoms, and health history to decide what’s appropriate for you.

Is every formulation appropriate for me?

No. Your provider selects a formulation based on your individual symptoms, history, and goals, not every option listed is appropriate for every patient.

Is this available in my state?

Availability varies by formulation and state. Your provider will confirm what can be prescribed and shipped to you during your consultation.

Can I use my HSA or FSA?

Yes. Medura accepts HSA and FSA cards at checkout, and we can provide documentation for reimbursement if you need it.

What happens after I request treatment through Medura?

You complete an online assessment about your health history, current medications and goals. A licensed provider reviews it and decides whether a prescription is appropriate. Completing the assessment does not guarantee a prescription. If one is written, a licensed pharmacy prepares and ships it, and your provider can adjust or stop treatment as needed. If lab work is needed, your provider will tell you; it may be ordered and billed separately.
